Was I hacked?

I received an email explaining that I was in violation of their TOS.

A network security engineer for a small bank has claimed that my domain/ip is stealing their customers login information.

I did not see what was on the pages that was doing this because Knownhost removed them but it was setup with this path:

new/reg/3/regions/index.html
new/reg/2/regions/index.html

I don't know how these files got on my server. How could this have happened and how can I prevent it?
